[Ctk-developers] Updating the "The Team" page

Hi again,
I now implemented most of the discussed changes to "The Team" page. Here
is a complete summary:
Founders
--------
- Renamed from "Pro tempore steering committee"
- Moved it to the top of the page
Supporting institutions
-----------------------
- Renamed from "Institutions"
- Added the names of group leaders/division heads who are/were involved
in supporting CTK activities. Please verify that the names, titles,
and affiliations are correct.
- Added The University of Sheffield (due to some relocations of Alejandro
Frangi from Barcelona to Sheffield).
Individual contributors
-----------------------
- New section, listing the contributors to the CTK code base.
- This is a snapshot of the Ohloh data, not yet using the Ohloh API.
- If I find the time, I will try to implement a proper Ohloh API query.
Until then, I will update that list manually after each hackfest.
Grants, Fundings and Sponsors
-----------------------------
- Added a German project supporting the CTK interoperability efforts.
I hope everybody is happy with the changes. Let me know if there is
missing/incorrect data.
