[ITK] Conversion from ITK to VTK coordinate spaces

Hi Sam,

your question isn't totally well defined. Just the matrix would be the
same. If you are dealing with oriented images, this example
<http://www.vtk.org/Wiki/ITK/Examples/WishList/IO/itkVtkImageConvertDICOM>
might be handy.
