I don't know about vtk itself, but in the slicer (www.slicer.org) code we
have a class called vtkImageConnectivity which does this.  It expects
short input and it outputs a unique label in each connected region. It
also has other functionality like island measurement.  The class is used
in the Editor module of the slicer.
