[vtkusers] vtk.h file in nightly release

vtk.h is generated intermittently by hand. when we write apps, we explicitly include the class header files that we use. if you include vtk.h, your compile times will be very slow.

this, however, is a poor excuse for our sloppy work. we''l look into an automatic generator for the include file,
