[Paraview] there may be a bug in paraview 3.2.1

I can confirm that your program seems to produce reasonable output in
VTK format but ParaView from CVS can not read the output in MHD format.

It looks to me like this program is just the ITK ThinPlateSplineWarp.cxx
example adapted for 2 dimensions, is that correct?
