Author & Ownership

I have marked places that needs update with review comments (in red). You can also search for comments using keyword "RComment".
I have fixed most of the small places, such as Dart->CDash, phpBugTracker->Mantis Bug Tracker; some of them still need update figures.

I have assign the following owner ship to the chapters. Chapter numbering based on the latest version.
